Health information

As an important note: now is not too early to begin scheduling freshman school physicals. All students entering 9th grade are required to have a school physical prior to August 1, 2024. Students will not be able to start school until the physical is completed. You can access the physical form by clicking here. Completed forms can be scanned to: Health.Forms@d300.org or delivered to the nurse at your student's current school.

High School Transition Nights

Dundee-Crown High School, Hampshire High School, and Jacobs High School will host a High School Transition Event on January 22, 2024 starting at 6:00 p.m. at each high school campus (please note, High School Transition Events will be held on the same date and time at each high school). Families should attend the High School Transition Night at the school the incoming freshman will attend.
