Make a Report
-
The goal at D300 is to make reporting sexual misconduct, dating violence, stalking, or retaliation as easy and comfortable as possible.
We have several options for students and/or guardians:
You can report directly to the Title IX Coordinator, Colleen O’Keefe by completing the Title IX Complaint form, or leave a voicemail message at the Title IX Complaint Hotline: 847-551-8321.
You can call the Police Department at: Algonquin (847-658-4531); Carpentersville (847-551-3481); Dundee (E) (847-428-4034); Dundee (W) (847-551-3810); Hampshire (847-683-2240).
You can tell a Principal, Social Worker, Counselor, or any other school employee, either verbally or in writing. All employees at D300 are “mandatory reporters,” which means that all employees are required to report these instances to the Title IX Coordinator, and in some cases, the Illinois Department of Children & Family Services.
