Health Requirements for the 2019-2020 School Year

    • All students must be up-to-date with physical examination and immunization requirements. All forms must be turned into the School Health Office prior to August 1, 2019 for the 2019-2020 school year to ensure students can begin school on the first day of attendance. Students will not be able to attend school until all required health information is on file.

      All students entering Early Childhood, Kindergarten, 6th grade, and 9th grade are required to have a current physical examination completed on the approved Illinois Department of Public Health Form. A sport physical does not meet the requirement. A current physical is one that was completed within one year of August 2019. The parent portion on the upper back side of the form must be completed and signed. All students must be up-to-date with the State of Illinois immunization requirements.

      Immunization records can be viewed through the Infinite Campus Portal. Physical examination forms are available in all School Health Offices and on the District 300 Health Services webpage.

      District 300, together with Advocate Childrens Ronald McDonald Charities, offer physical and immunization clinics onsite at District 300 schools throughout the year. These clinics provide the required school physicals and immunizations for students with Medicaid as their primary insurance.       Requirement for School Entry Illinois Department of Public Health School Year 2019-2020

      Now is not too early to begin scheduling important physical and immunization appointments! All students must be up-to-date with physical examination and immunization requirements and all forms must be turned into the School Health Office prior to August 1st to ensure students can begin school on the first day of attendance. Students will not be able to attend school until all required health information is on file.

       

      Important Health Requirements 2019-2020

      All students entering Early Childhood, Kindergarten, Sixth and Ninth grades are required to have a current physical examination completed on the approved Illinois Department of Public Heath Form. A sport physical does not meet the requirement. A current physical is one that was completed within one year of August 2019. The parent portion on the upper back side of the form must be completed and signed. All students must be up-to-date with the State of Illinois immunization requirements including the following:  

      • All students entering sixth, seventh, eighth, ninth, and tenth grade must show proof of receiving one dose of Meningococcal vaccine (meningitis).
      • All students entering twelfth grade must show proof of receiving one dose of Meningococcal after the age of 16.
      • All students entering grades six through twelve must show proof of receiving one dose of Tdap.

      Dental Examination Requirement

      All students entering kindergarten, second, sixth, and ninth grades are required to have a dental examination. Dental examinations are due by May 15th of the year the student is enrolled in kindergarten, second or sixth grade. A current examination is one that was completed within 18 months of May 15th.

       

      Eye Examination Requirement 

      All students enrolling in a kindergarten program or enrolling for the first time in a public school shall submit proof of an eye examination completed within the previous year before October 15th of the school year.

       

      Hands Only CPR

      District 300 takes seriously the health and safety of all students, staff and visitors who are on school premises. All District Schools are equipped with Automated External Defibrillators (AED's) and each building has staff members that have been trained in the use of these devices. Opportunities are also made available throughout the year for staff and coaches to receive Cardiopulmonary Resuscitation (CPR) and AED training. District 300 encourages staff, parents and students to become familiar with Hands Only CPR and the use of an AED.
