Students entering grade 12 in the State of Illinois must be immunized for Meningitis. The immunization requirement consists of two doses of the MCV4 vaccine for entry into 12th grade. Only one dose of the MCV4 is required for students entering 12th grade if the first dose was received on/after the age of 16.

Proof of receiving the MCV4 Vaccination(s) is required prior to August 1, 2024. Students without proof of receiving the MCV4 vaccination will be excluded from school until such time that they present proof of receiving MCV4.
