Student Health Requirements Due by October 15, 2021

All District 300 students must be up-to-date with their physical examinations and immunization requirements, and all forms must be turned in by October 15, 2021. Please disregard this section if you previously submitted your child’s physical examinations and immunization requirements forms during the summer (or earlier this school year).

Students missing required physical examinations and/or immunizations will not be able to attend school after October 15 until all the necessary health information has been submitted to the health office.

Please note: the immunizations referenced in this section are NOT related to COVID-19 vaccines. Instead, these are the immunizations that have historically been required at the beginning of each school year.

What Grade Levels Are Required Submit Health Forms by October 15, 2021:

  • Preschool (if this is your child’s first year in preschool) 
  • Kindergarten
  • 6th Grade
  • 9th Grade
  • 12th Grade

All students in the grades listed above are required to have a current physical examination completed on the approved Illinois Department of Public Health Form with all required immunizations. A sports physical does not meet the requirement. A current physical is completed within one year of October 15, 2021.

  • Preschool Students: Must complete a physical examination

  • Kindergarten Students: Click here to see the kindergarten health requirements (deadline extended from August 1 to October 15)

  • 6th Grade: Complete 6th-grade Physical Examination and all required immunizations, including:
  • One dose of Tdap Vaccine

  • One dose of Meningococcal Vaccine (meningitis) after the 11th birthday 

  • Two doses of Varicella Vaccine (chickenpox)

  • Three doses of Hepatitis B at the required intervals
  • 9th Grade: Complete 9th-grade Physical Examination and all required immunizations.

  • 12th Grade: Must show proof of receiving one dose of Meningococcal (MCV4) vaccine on/after the age of 16.

How To Submit Health Requirement Documents:

Parents and/or guardians may submit documentation of physicals and/or immunizations/vaccinations by emailing Health.Forms@d300.org, faxing 847-551-8433, or in-person at your school’s main office.
