Required Physical Examinations – A wellness clinic for physical exams and immunizations will be held on-site during registration. Location and times will be determined and posted soon.
Physical examinations completed within one year prior to the date of entrance are required for all students transferring from out-of-state schools as well as those entering the following grade levels/programs: Early Childhood (first year), Kindergarten, Sixth Grade, and Ninth Grade.
The Illinois Department of Public Health requires that students show evidence of having received immunization for diphtheria, pertussis (whooping cough), tetanus, polio, mumps, measles and rubella. State law requires all students, K-12; show proof of receiving two doses of the measles vaccine. Early childhood and all students in grades 5-12, must show proof of three doses of hepatitis vaccine at the appropriate intervals. One dose of the varicella (chicken pox) vaccine is mandated for early childhood through grade 8.
ALL PHYSICAL EXAMINATION FORMS AND PROOF OF IMMUNIZATIONS MUST BE COMPLETED AND RETURNED TO THE SCHOOL NURSE NO LATER THAN THE FIRST DAY OF ATTENDANCE. THE FIRST DAY OF ATTENDANCE FOR THE 12/13 SCHOOL YEAR IS AUGUST 13, 2012.
A dental examination is also required for: Kindergarten, Second grade and Sixth grade
Dental examination forms are due by May 15 of the year the student is enrolled in Kindergarten, Second and Sixth grades.
A vision examination is also required for all students entering kindergarten or enrolling for the first time in a public, private or parochial school. Vision exam forms are due by October 14.
