D300 with Advocate Children’s Healthcare and Ronald McDonald Charities to Host Physical and Immunization Clinics

As a reminder, all incoming Early Childhood (first time students), Kindergarten, 6th grade, 9th grade, and 12th grade students must be up-to-date with physical examination and immunization requirements before August 1, 2017. 

All health documents for Early Childhood, Kindergarten, 6th, 9th, and 12th grade students must be turned into your school’s main office by August 1, 2017 to ensure students can begin school on the first day of attendance.  Students will not be able to attend school until all required health information is on file.

All students entering Early Childhood, Kindergarten, 6th, and 9th grades are required to have a current physical examination completed on the approved Illinois Department of Public Health Form.  A sports physical does not meet the requirement. A current physical is one that was completed within one year of August 2016. 

All students entering Early Childhood, Kindergarten, 6th, 9th, and 12th grade must be up-to-date with the State of Illinois immunization requirements. Parents can see the full list of health requirements below:

Early Childhood Students:

Complete Early Childhood Physical Examination (if entering Early Childhood for the first time) and all required immunization including:

  • Three doses of DTP/DTaP
  • Two doses of Polio Vaccine
  • One dose of MMR
  • Hib Vaccine
  • Pneumococcal Vaccine
  • One dose of Varicella
  • Three doses of Hepatitis B Vaccine at the appropriate intervals

Kindergarten Students: 

Complete Kindergarten Physical Examination and all required immunizations including:

  • Four doses of DTP/DTaP
  • Four doses of Polio Vaccine
  • Two doses of MMR
  • Two doses of Varicella Vaccine (chicken pox)
  • Complete Kindergarten Dental Examination– Must be turned in prior to May 15th 2018
  • Complete Kindergarten Vision Examination– Must be turned in prior to October 15th, 2017

6th Grade Students

Complete 6th Grade Physical Examination and all required immunizations including the following:

  • Three doses of DTP/DTaP
  • One dose of Tdap Vaccine
  • Three doses of Polio Vaccine
  • Two doses of MMR
  • One dose of Meningococcal Vaccine (meningitis)
  • Two doses of Varicella (chicken pox)
  • Three doses of Hepatitis B Vaccine at the required intervals

Complete 6th Grade Dental Examination – Must be turned in prior to May 15th, 2018

9th Grade Students

Complete 9th Grade Physical Examination and all required immunizations.

12th Grade Students

Must show proof of receiving two doses of meningococcal vaccine (meningitis) unless the first dose was given after the age of 16, then only one dose is required. (12th grade students are not required to receive a physical examination.)

Back to School Clinics Information:

District 300, together with Lutheran General/Ronald McDonald Charities, are offering Back to School Physical and Immunization Clinics for students with Medicaid as their primary insurance or for students that have no health insurance coverage.  Private insurance or fees for services can’t be accepted through this grant program. Clinics will be offered on the following:

July 18th 8:30-3:00 @ Carpentersville Middle School

July 20th 8:30-3:00 @ Dundee Crown High School

July 25th 8:30-3:00 @ Carpentersville Middle School

July 27th 8:30-3:00 @ Dundee Crown High School

August 7th 8:30-3:00 @ Hampshire High School

These Physical and Immunizations Clinics are for students who have Medicaid as their primary insurance or for those students with no insurance. Parents must make an appointment for students to receive immunizations or physicals. Please call 847-551-8371 , to schedule your appointment for any of the dates listed.
